2. Requested Funding Tier*

   1. Tier 1: less than $2500 (No letter of intent required)
   2. Tier 2: less than $10,000
   3. Tier 3: less than $50,000
   4. Tier 4: special project above $50,000

   Please check the box indicating which funding tier you intend to apply for if letter of intent is approved by the LADPC board.

4. Letter of Intent*

   Please upload a PDF file of a signed letter of intent. This will be reviewed by the LADPC board. The board will determine if your project meets LADPC requirements (ORS 430.355). You must receive approval to proceed to a full application if applying for funding tiers 2, 3, or 4 (more than $2,500)
